# How do I make a manual sample form? updated

I am trying to make a manual sample entry form however I am stuck after I add the location selector and then definition selector.

I am stuck on linking the attributes to the sample entry forms.  
Edit  
According to the docs the form is made dynamically but I cant seem to generate one. I am probably overthinking it

I have

select location -\> select definition then i used a dropdown to find the attributes to measure but now Im stuck

I would prefer without heavy scripting.
