I noticed today that I cannot add any phone contact information to users both in the client user management components or the user management section on the gateway web interface. When I add a new contact option I am only given two items in the list: email and SMS. I don’t even have the SMS module installed so it looks like something is messed up there.
Can you post a screenshot of your modules page?
You know what… I just noticed that the phone module didn’t get upgraded and is still at 1.6.3. I bet that’s probably the problem.

That was the issue. I upgraded the voice notification module to 1.6.6 and restarted the server and now I can add phone information again. My bad, thanks!
