How risky is it to change a device name on a very mature gateway running 7.8.3?

It will break all the tags pointing at that device. Plan on exporting all of your tags to xml, running an intelligent search and replace on that device name, and importing when you are ready to actually make the change. If you are doing any system.opc.* calls with that device, you will have to make simultaneous changes to your scripts.

You’d have to update all the OPC Item Paths in tags from that device as well.
