How to create 'Module Configuration Page'?

I started developing an OPC UA driver module.
But in the documentation, I could not find how can I add a 'Module Configuration Page'.
Even in opc-ua-device example source code.

I want to have a page like the following picture:

I think @pturmel has had good experiences in the ' Advanced Modbus Driver' project. Please help me.

It's wicket. It's a steep learning curve. No immediate help for you. Will review what I can share later this week.

I need a sample code like even deprecated "Modbus driver example".
I want to speed up my learning by reverse engineering.
