It means the OPC Server lost connection with an OPC Client that was connected. Can't really say anything more from just that log message.
we have only two connections. One of them is a loop back connection with the gateway itself and a 3rd party OPC server. Is it possible to see which one? Is it possible that the loop back connection itself is faulty?
It's possible, yes. You'd probably see its status as faulted unless it recovered right away.
Otherwise it's another client connected to the Ignition server, or some other thing on the network like a security scan temporarily connecting, or something.
