Hi, Its showing opc connection faulted. Please I need your help.

Just pasted your code into preformatted text to help read it, but have you changed anything with either the user sources or OPC settings? Out of the box with no modifications evrerything should just work, but if you changed the OPC user source, OPC client connection, or the OPC server settings, you may have broken it. More details will be needed and possibly screenshots to see how you have everything set up.
