Transferring certification to a new employer

Currently I’m Gold Certified under the name of my previous employer who released me when I was working on the exam. The company shows as a Gold integrator, presumably only because of my passing of the exam.

What is the procedure to attach the credential to a new employer or give it “freelancer” status? Can more than one entity be attached at a time?
